To request your initial assessment, you must review the information below and be certain you understand your payment responsibilities. While we can accept some insurance plans, not all will cover us. If your insurance will not cover your appointment, you will be responsible for payment in full at the time of service.

Payment Options
Limited Insurance are Accepted for Direct Billing:BCBS PPO, Anthem PPO, Cigna PPO, and Provider Network of America (PNOA) PPO insurance plans may be covered at 100% for office & home infant and toddler lactation visits and telehealth through The Lactation Network! If you have one of these plans (your card will specifically say PPO on it if you do), please click the link below to get prior authorization. Until you complete this step, we cannot bill your insurance!
*There may be limitations within these plans. Coverage is dependent upon PRIOR AUTHORIZATION only.
If your insurance covers standalone IBCLC services:If your insurance doesn't allow us to bill them directly, you may still have coverage! You should call your insurance company directly to confirm their coverage of private-practice IBCLC coverage before choosing this option. In this situation, a "super bill" will be provided to you after you've paid for your lactation services so that you can seek your own reimbursement from your health insurance provider. Some families receive 100% of what they pay back, while others may be reimbursed at your plan's out-of-network percentage rate. We make no guarantee of reimbursement from your plan, as these payments are made at the discretion of the insurance company, in accordance with your (or your baby's) individual benefit plan so we highly recommend that you confirm directly with your insurance carrier before you come in for your first appointment. A further discount ($50 off initial assessments and $20 off follow-up appointments) is available for recipients of Medicaid, as this is one type of insurance that will never reimburse for our services... but please contact your elected representatives to request that they can help IBCLCs can become licensed in the state of NY so that we can be reimbursable for everyone!

We strongly believe that we have a responsibility to our clients as well as to the public at large to help spread our knowledge and expertise of complex lactation support, tethered oral tissues, and orofacial functional development to other providers. As such, you may have a visiting professional or apprentice IBCLC present during your appointment. You ALWAYS have the right to refuse any additional observers in the room who do not work directly with WNY Orofacial.
Virtual and Home-based Appointments are also available! Please reach out to discuss pricing and availability.
We highly recommend that families with multiple children to be assessed and/or treated to keep in mind that it will take longer to provide each of your children the same high-quality services, so please schedule a separate appointment for each child. We can not assess multiple children during the same appointment slot.
